Job Description:

A company is looking for an I/DD In-Reach & Transition Coordinator to support individuals with Intellectual Developmental Disabilities in transitioning to community-based services.



Key Responsibilities

Identify consumers/enrollees in State Developmental Centers and ICFs for potential community placement
Facilitate the transition process by coordinating housing options and support services for individuals
Document progress and maintain clear communication with stakeholders involved in the transition planning


Required Qualifications, Training, and Education

Bachelor's degree in public service administration, psychology, sociology, social work, or a related field with four years of relevant experience, or a Master's degree with two years of supervised clinical experience
Experience in case management, assessment and referral, supportive counseling, and treatment planning
Direct experience working with children, adolescents, and adults with mental health and intellectual/developmental disabilities
Knowledge of medical, behavioral, and psychosocial issues relevant to the population served
Ability to work effectively in a multi-disciplinary team environment
