You coordinate the plan of care with other health care professionals and help maintain patient care continuity.Key Job Duties:
  • Provide high quality clinical services within infection control standards, in accordance with the plan of care and other team members.
  • Complete clinical nursing assessments according to federal and state program requirements.
  • Ensure patient eligibility and medical necessity for services as defined by payer source and agency policy.
  • Develop and revise individualized plans of care and/or service plans according to federal and state program requirements with other community providers.
  • Ensure plan of care frequency and duration meets patient needs and initiates revisions as needed with physician approval.Experience:
    • A minimum of one year experience as an RN is preferred.License Requirements:
      • Current RN licensure in state of practice.
      • Current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation.
      • Current CPR certification is required.
